Helping clients with bathing, dressing, grooming, toileting, and oral hygiene
Assisting with mobility such as walking or transferring from bed to chair
Supporting continence care and changing incontinence products
Nutrition and Meals
Assisting with meal preparation, serving, and feeding
Monitoring food and fluid intake and reporting concerns to supervisors
Following dietary plans or restrictions as directed by care staff
Medication Assistance
Providing medication reminders or assistance per MAR (Medication Administration Record)
Ensuring compliance with agency policy and training (note: health care aides do not administer medication unless authorized)
Homemaking and Environmental Support
Performing light housekeeping tasks such as laundry, dishwashing, and vacuuming
Maintaining a clean, organized, and safe home environment
Supporting daily routines and encouraging client independence
Emotional and Social Support
Offering companionship, meaningful conversation, and emotional comfort
Supporting clients in community activities or escorting them to appointments
Observation and Reporting
Monitoring clients for changes in condition, mood, or safety concerns
Reporting observations promptly to supervising nurses or coordinators
Documenting care activities accurately according to agency or WRHA policy

Observing changes in condition, mood, or safety
Reporting updates promptly to supervising nurses or physicians

One-Way Transportation

Two-Way Transport: Round-trip transportation to and from your appointments or scheduled activities.
Appointment Escort: A dedicated caregiver accompanies you during your appointment to provide support and assistance as needed.
Hospital Discharge Transport
Hospital Discharge Transport: Comfortable and reliable transportation from the hospital back home, with help settling in safely.
Anytime Transport
Overnight Transport: Assistance for clients requiring transportation or accompaniment during any time.

Self and Family Managed Care is a flexible home care option offered through the Winnipeg Regional Health Authority that allows eligible clients or their families to take greater control of personal care services. With approved funding and a completed home care assessment, clients can choose their caregivers, set schedules, and manage care in a way that best supports independence, comfort, and quality of life at home.

We understand that paying out of pocket can be difficult, especially after experiencing a major health incident. For this reason, we offer the option to bill directly to your insurance provider, helping ease the financial burden during your recovery
